Conversations about end of life care

Title: Conversations about end of life care in heart failure and Chronic Obstructive Pulmonary Disease: systematic reviews
Principal research question: With regard to the adult patients with life-limiting illnesses discussing End of Life Care issues with health professionals:
1) What is discussed at present, and when?
2) What would patients like to discuss and when?
3) What do health professionals think they should discuss, and when?
4) What are the cues / barriers / facilitators of these discussions?
Methodology description: Systematic literature review and Narrative Synthesis. Search of Medline, PsycINFO and CINAHL databases, citation searches and journal hand searches.
