Occupational Health Form
If you successfully complete Stage 2 of the application process you will be asked to attend an Occupational Health Appointment on the first day of your placement for immunisation screening. This is in accordance with the Department of Health (DOH) guidance for standard communicable disease screening checks required for all new NHS healthcare workers. The screening checks are outlined on the Occupational Health page of our website. This ensures that all students are adequately protected to undertake clinical activities, minimising infection risk to both patients and staff. For more information about Occupational Health please visit the Occupational Health page on our website.
In preparation for this the University Occupational Health Service requests that all students complete and return an Occupational Health Form prior to their placement.
Download your Visiting Elective Medical Student Health Assessment form, complete it and return directly to Occupational Health at the email address listed below:

If you have any queries regarding the forms or Occupational Health, please contact the OH team directly, as they are the best people to answer questions in this regard.
Your form must be received by the Occupational Health Service by the date specified in your offer letter. Please be aware that the OH team is very busy and can sometimes take longer than expected to confirm safe receipt of your form. Delays on their part will not affect your application.
